Healthy Homestead offers client orientated services specifically designed to support high-needs children and adults. Our mission is to provide professional, integrated care that fosters community inclusion and improves quality of life for every individual we serve.
Day Habilitation
- Who it’s for: High-needs children, youth and adults requiring structured peer engagement and skill development outside the home.
- What happens: Day Habilitation programs provide structured, engaging, and person-centered opportunities for individuals to develop life skills, build meaningful relationships, and actively participate in their communities. By building on each person's unique strengths, interests, goals, and support needs, these programs promote greater independence, overall wellbeing, and a sense of purpose. Through meaningful daily activities, individuals are empowered to enhance their physical, mental, emotional, and social health while fostering personal growth, community inclusion, and lifelong learning.
- Typical schedule: During the intake process, program days and times are determined collaboratively based on the individual's needs and preferences, while considering family schedules and staff availability. This flexible approach helps ensure that services are person-centered, accessible, and responsive to each participant's unique circumstances.
Family Habilitation
- Who it’s for: Individuals who would benefit from living in a licensed assisted living or licensed foster home where a trained caregiver, who is not a member of the individual's immediate family, provides individualized, person-centered support in a safe, stable, and family-style environment.
- What happens: Individuals receive daily support tailored to their unique strengths, goals, and support needs. Services focus on developing life skills, promoting independence, fostering meaningful relationships, encouraging community participation, and supporting overall physical, mental, emotional, and social wellbeing. Care is provided in collaboration with families, guardians, and interdisciplinary care teams to ensure a coordinated, person-centered approach.
- Typical schedule: Family Home Habilitation is a residential service in which the individual lives full-time in the licensed home. Supports are provided daily and are individualized to meet the person's needs, goals, and routines while promoting a safe, supportive, and enriching home environment.
In-Home Supports
- Who it’s for: Children, youth, and adults with intellectual or developmental disabilities who require professional assistance to develop independent living skills while residing in their family home or community setting.
- What happens: Our compassionate staff provides 1:1 support tailored to the individual’s goals, focusing on personal care, household management, community navigation, and safety skills. We work closely with families to ensure routines are respected while fostering the individual's growth toward greater self-sufficiency and emotional wellbeing.
- Typical schedule: Services are scheduled based on the authorized support hours and the family's specific needs, ranging from a few hours a week to daily visits, ensuring a seamless integration into the individual’s natural environment.
Respite Services
- Who it’s for: Primary caregivers and families of children or adults with complex needs who require a professional, short-term break while ensuring their loved one receives continuous, specialized care.
- What happens: Respite provides a safe and engaging environment for the individual, focusing on social interaction and recreational activities. This service offers families peace of mind, knowing their loved one is in the hands of trained professionals who prioritize safety, health, and personal enrichment.
- Typical schedule: Respite hours are flexible and can be used for planned breaks or emergencies. Whether for a few hours or a scheduled period, our team coordinates with the family to provide consistent support that aligns with the individual’s routine.
Nurse Oversight Case Management
- Who It's For: Nurse Oversight and Case Management (NOCM) services are designed for individuals with complex medical needs who require extraordinary supervision, observation, and specialized medical support to safely remain in their home and community. This service is available to eligible individuals, including children with complex medical conditions and individuals with developmental disabilities who meet program requirements.
- What Happens: A registered nurse provides individualized medical oversight, care coordination, and clinical support based on each person's unique health needs and service plan. The nurse develops and implements a nursing plan, provides training and guidance to individuals and caregivers, and ensures that necessary medical care tasks are performed safely and effectively.
- NOCM services may include:
- Developing and monitoring individualized nursing care plans.
- Training individuals, families, and caregivers on specialized medical care procedures.
- Coordinating communication between healthcare providers, caregivers, and support teams.
- Monitoring health needs, medical equipment, and treatment routines.
- Supporting safe and effective delivery of care within the home and community.
- Reviewing services and adjusting supports as medical needs change
- Typical Schedule: NOCM services are provided according to the individual's medical needs, nursing plan, and approved service authorization. Registered nurses provide ongoing oversight, monitoring, caregiver training, and care coordination to support continuity of care and promote the individual's health, safety, and quality of life. NOCM nurses visit the clients at minimum of once a month and provide training/support as needed.
At Healthy Homestead, our NOCM team works collaboratively with individuals, families, caregivers, and healthcare providers to ensure medically complex individuals receive the support needed to thrive in their homes and communities. We help advocate for needed DME and services through the Medicaid system.
Foster Parent Support
- Provides specialized training and guidance to foster homes caring for high-needs youth.
- Ensures home stability through compassionate professional oversight and regular check-ins.
- Empowers foster parents with resources for confidence, stability, and child-centered care.
